Most Improved Stroke Service

The Stroke Association Life After Stroke Awards are held each year to celebrate the courage and determination of stroke survivors in overcoming the physical, emotional and communication difficulties as a result of stroke.

Additionally, the awards have a range of categories that also recognise and celebrate the work of individual health and social care professionals, volunteers, clubs and carers.

The awards have been successful in highlighting the improvements that have been made to stroke services that have benefited the lives of stroke survivors.

This year we are pleased to announce that the Life After Stroke Awards include a new category enabling us to recognise the Most Improved Stroke Service. The award will be presented at the Life After Stroke Awards ceremony at Claridge's on 15 June.

If you feel your Stroke Service deserves to win this prestigious award please nominate them!

Nominations close on 30 April 2010. Nominations should be no longer than one typed A4 page and need to outline the following information:

  1. Where is the service located?
  2. When did the service start?
  3. How many staff members are there? What are their roles?
  4. In the last 12 months, what have been the service's most significant achievements and improvements?
  5. In what ways does the service enhance the lives of people affected by stroke in the local area?
  6. Please give specific examples of how the stroke service has gone above and beyond regular levels of service provision to provide excellent service for stroke survivors.
  7. Please summarise in 250 words why you think the stroke service should receive the Most Improved Stroke Service award.


You also need to provide us with your contact details and the contact details for the stroke service you are nominating (including email addresses and phone numbers). This does not count towards your one typed A4 page limit.
