Social Care in the East Midlands
Support services to help the person who has had a stroke manage at home are usually arranged through the local authority social care department. The person may have already seen a social worker in hospital. If not, their GP can refer them or they can contact the department themselves.
With improvements to stroke care driven by the National Stroke Strategy, social care departments now have a time-restricted pot of money dedicated to stroke service improvement.
Social care departments will assess the individual care needs of people affected by stroke and their carers. They will then match those identified needs with the services that they offer.
