Completing the application form - guidance notes

Please use this guide to help you complete the application form. The details you provide will be used during the shortlisting process and, as appropriate, the relevant stages of the selection process.

General points
• Your application should be completed in black ink or typescript.
• Please state the post that you are applying for on all documents you submit.
• If you require assistance to complete the form, please contact the Human Resources Department on 020 7566 0325, 020 7566 0302 or email the Recruitment Team

Education details and formal qualifications
• In addition to providing information regarding your academic qualifications and achievements, please provide details of any professional development that you have undertaken and membership of any appropriate professional organisations.
• Some posts do not necessarily require formal qualifications. Please refer to the person specification for clarification.

Employment history
• Please provide details of previous jobs held and account for any gaps in your employment history.
• You may have developed relevant skills through unpaid work, which could include raising a family or voluntary work. Please include these details on your application.

Additional information to support your application
• You are asked to demonstrate why you are suitable for the post. Before completing this section, please refer to the job description and person specification.
• Ensure that you provide specific examples to substantiate your supporting statement. This means including what you did in your job, rather than what you would do or what the team did. You should provide examples of where and how you demonstrated a particular skill, instead of simply saying that you have the skill. Address all of the points identified on the person specification, as best as you can.
• Make sure that the information you provide is relevant to the job, and please be honest.

References
• Please provide the names of two people from whom references may be sought. One of these should be your present/last employer, if possible. References will not be taken up until a provisional offer of employment has been verbally accepted.
• If you are unemployed, your last employer should be provided as a referee. If you have had voluntary or unpaid work, you should include a contact within that organisation for reference purposes.
• Where you are only able to provide the names of friends as referees, please continue to complete the rest of the application and indicate the reason for this.

Personal details form
• The Stroke Association is committed to equality of opportunity. To support this applicants are asked to supply information relating to their ethnicity, gender, sexual orientation and age, and whether they have a disability, as defined under the Disability Discrimination Act. This information is anonymous and provided for statistical and monitoring purposes only. This information is requested in the Personal Details Form.
• Applicants who request that an application pack is sent to them in the post should return the personal details form in the enclosed pre-paid envelope.
• Applicants who complete the personal details form electronically should email the form to the Recruitment Team


Returning your application form
• It is advised that you photocopy your form before returning it to us, as you may be asked questions at the interview about the information you have supplied.
• Applicants who request that an application pack is sent to them in the post should return their completed form to the address shown on the back of the application form.
• Applicants who complete the application form electronically should email their completed form to the appropriate email contact, as detailed in the advert.
• Your form should be returned by the published closing date. If your application arrives after the published closing date and no prior approval has been granted, The Stroke Association reserves the right not to include your form in the shortlisting process.

Finally
The Stroke Association can only assess your suitability for the job from the information that you provide on your application form.
