Living with stroke starts after discharge, but the transition from hospital to home can be extremely traumatic. (National Audit Office) The Community Integration Service is designed to provide an opportunity to identify and pursue realistic social and occupational opportunities within the community for all stroke survivors. Stroke survivors need continuing support and community care as they work towards regaining confidence, skills and independence after their stroke. As the leading specialist provider of stroke services, we have established a comprehensive quality improvement programme that ensures quality across all our services.
