People affected by stroke require a seamless transfer of care comprised of a workable, clear discharge plan that has fully involved the individual and their family where appropriate. (National Stroke Strategy)

The Family and Carer support service is designed to provide information, emotional support and practical advice to people affected by stroke, their families and carers. 

Our staff work alongside a multidisciplinary team of professionals, across health and social care, to provide information and advocacy support for stroke survivors and their families as they work towards regaining independence.
