Life After Stroke Services

For over 20 years, The Stroke Association has been working to provide specialist stroke rehabilitation and support services to the stroke community in the UK.

In order to meet the ever-changing needs and expectations of stroke survivors, their families and carers, we have recently developed a new model of long-term care that we are calling Life After Stroke.

Our model of long-term stroke care
The Life After Stroke Services model places the person affected by stroke (our client – whether stroke survivor, family member or carer) at the heart of the entire care pathway.

A key worker, the Information, Advice and Support Co-ordinator, bridges the gap between health and social care by offering a clear, consistent point of contact for the client from hospital through to returning to the community.

The co-ordinator helps the client to navigate the health and social care system, making sure they remain supported after returning home from hospital and able to access all the services available to them.

With five categories of service provided by The Stroke Association, the model encompasses the full range of long-term needs of those affected by stroke, every step of the way – from communication support to a Black and Minority Ethnic Service to returning to work.

The model is designed to make it as easy and affordable as possible for health and social care commissioners to provide a comprehensive, high quality and client-centred stroke service in your local area.
