Information, Advice and Support Service
Our Information, Advice and Support Service provides stroke survivors, families and carers with high quality information, emotional support and practical assistance in the aftermath of a stroke. We work closely with individuals and families to make sure their needs are being met and help them come to terms with the prospect of life after stroke.
A key part of the service is helping people to find their way around the system. Our co-ordinators will direct clients towards other services that can meet their needs and help them achieve their goals. We seek to develop strong links with all the sources of support available, including statutory services and voluntary organisations.
The Information, Advice and Support Service is one of our key programmes, and is usually the first step in our Life After Stroke pathway. We help to set people affected by stroke on the road to recovery by offering continuous support, from hospital to home and into the community.
Outcomes
• People come to terms with what has happened and find a way of living with life after stroke
• Stroke survivors and their families given more choice and control over their care
• People find their way around the care and support system, which works more efficiently
• Shorter hospital stays and fewer readmissions as people are supported through discharge
• Improved health and fewer subsequent strokes
• Seamless transfer of care, joining up hospital and social care services
• More people able to live independently and to pursue their goals
• Carers feel supported and encouraged, and provide better care as a result
