Reassessment and discharge
Stroke is an ongoing condition. Needs change, goals shift.
Our co-ordinators keep in regular contact with clients to reassess their situation and make sure they’re still receiving the right support.
If necessary, our co-ordinators will point people to other services, or refer them back to stroke professionals such as consultants and therapists.
Some clients will require no further support and will be ready to move on, though many take up the opportunity to give something back as volunteers. Others will continue to take part in stroke groups or need long-term support.
