Stroke prevention services

Our Stroke Prevention Services work with people most at risk of having a stroke – for example those who have already had a stroke or TIA, or who have a history of cardiovascular problems.

The services could also benefit those who have been identified as having a high risk of stroke through the vascular checks programme. We work intensively with individuals and their families to help them make lasting changes to their lifestyles to reduce the risk of stroke.

Our Stroke Prevention Co-ordinator conducts one-to-one sessions with people in their own homes to identify and explain the stroke risk factors in their life. Together, they plan realistic lifestyle changes that will reduce their risk of having a stroke – such as giving up smoking, taking gentle exercise or eating less salt.

Along with individual sessions, we offer weekly healthy lifestyle groups. These include exercise classes and discussions of subjects such as cholesterol, taking the right medication and eating a healthy diet. 

Our services are based on each individual’s needs. A typical programme might consist of monthly individual sessions for nine months. Our strong links with other health, care and social services means co-ordinators can refer clients on to other services where appropriate.

We can easily adapt our services to meet local needs – for example, by running a programme that focuses specifically on exercise and fitness.
