Description

Our Post-Stroke Reviews service is here to help identify your needs. Our coordinator will contact you after your stroke to discuss our service, and to organise a phone call and/or home visit to find out how you’re managing with daily activities, and any additional support you may require.

This service offers:

A comprehensive review (approximately six months post-stroke) to identify any unmet needs or areas where you would like support;Emotional support;Shared decision-making and support planning;Support in accessing relevant information, such as My Stroke Guide, and signposting to relevant community organisations or clinicians;A blood pressure check.

Who is this service for?

This service is for stroke survivors (including TIAs/mini-strokes).

If you are unable to find a suitable service in your area, contact our Stroke Helpline on 0303 3033 100, or email helpline@stroke.org.uk, and we may be able to put you in touch with alternative support.
