About this service
Telford and Wrekin Post-Stroke Review Service is here to help identify your needs. Our coordinator will contact you after your stroke to discuss our service, and to organise a visit to come and find out how you’re managing with daily activities and any additional stroke support you may require. We’ll also make sure you know all about the services that can support you locally.
This service offers:
- a home visit to offer a comprehensive review approximately 6 months post stroke - this assessment will help you and us to identify any unmet needs or areas where you would like support
- a 12 month review to establish if there are any needs that are still unmet
- support in accessing relevant information, such as My Stroke Guide, and signposting to relevant community organisations
- a blood pressure check
Who is the service for?
This service is for stroke survivors living in the Telford and Wrekin area. The Stroke Association ensures to contact all stroke survivors in Telford and Wrekin soon after their stroke to arrange a six month post-stroke review.
