Description
Our Post-Stroke Reviews service will work with you to reflect on progress and identify any further support needs. Our coordinator will contact you to organise a phone call and/or home visit to find out how you’re managing with daily activities, and any additional support you may require.
This service is for stroke survivors.
Shared decision-making and support planning to identify any unmet needs or areas where you would like support;Emotional support;Support in accessing relevant information, and signposting or referring to other services in the community.
